Accessing federally funded home care packages

Here’s how:

  1. Find out if you’re eligible 
    To be eligible for subsidised homecare, you must first be assessed by the Australian Government Aged Care Assessment Team (ACAT). This straightforward process can be easily arranged by asking your GP or hospital discharge unit for an ACAT Assessment. If you’re eligible, your ACAT Assessment report will detail the level of government-subsidised homecare you’re entitled to, either:

    • Low-care – level 1-2 packages
    • High-care – level 3-4 packages
    • LGBTIQ-specific packages
  2. Meet with St Louis – they can come to you!
    So that St Louis can get to know you and learn about your specific care needs, they like to meet all clients interested in home care services. St Louis Care Coordinators are only too happy to come and visit your home to meet you and your family members or friends and talk about how both St Louis and Ubercare might be able to help you.
